These terms govern your use of this website, which is operated by Fit for Life Weight Loss Medical Centers, LLC, doing business as Fit 4 Life ("Fit 4 Life", "we"). By using the site or submitting the form on it, you agree to them.
Using this site
You may use this site to request an appointment at one of our offices. You agree not to use it for any unlawful purpose, not to interfere with how it works, and not to submit information that is false or that belongs to someone else without their permission.
Text message program
Program. Fit 4 Life appointment messages. If you check the optional SMS consent box on our form, you agree to receive text messages from Fit 4 Life about your appointment, including confirmations, reminders, arrival-time updates and rescheduling notices. Messages may be sent using automated technology.
Age requirement. You must be 18 years of age or older to use this SMS service.
Consent is optional. You do not have to agree to receive text messages to submit the form, to book an appointment, to buy anything from us, or to receive any service. If you leave the box unchecked, we will still contact you by phone or email, and we will not text you.
Frequency and cost. Message frequency varies with your appointments. Message and data rates may apply. Fit 4 Life does not charge for these messages; your carrier's standard rates apply.
To stop messages, reply STOP to any text from us. You will receive one confirmation message and nothing after that. To rejoin, check the box on our form again.

Carriers. Messaging is supported on the major US carriers. Carriers are not liable for delayed or undelivered messages.
Privacy. Mobile phone numbers and text message consent are never shared with third parties or affiliates for marketing or promotional purposes. See our Privacy Policy.

Everything on this site is general information. It is not medical advice and is not a substitute for a visit with a qualified healthcare professional. Requesting an appointment through this site does not create a doctor-patient relationship. If you have a medical emergency, call 911.

The content on this site, including text, images, logos and the Fit 4 Life name, is owned by or licensed to us and protected by copyright and trademark law. You may not reproduce, distribute or create derivative works from it without our written permission.

Governing law
These terms are governed by the laws of the State of Florida, without regard to its conflict of laws rules. Any dispute will be brought in the state or federal courts located in Miami-Dade County, Florida.
