Fit for Life Weight Loss Medical Centers, LLC, doing business as Fit 4 Life ("Fit 4 Life", "we"), respects your privacy. This policy explains what personal information we collect through this website, how we use it, who we share it with, and the choices you have. It applies to this site and to the calls, emails and, where you have opted in, text messages that follow from a form submitted on it.
Information we collect
When you submit our appointment request form, we collect what you enter:
- Your first and last name
- Email address
- Mobile phone number
- Your preferred office and the best time to reach you
- Whether you chose to receive text messages, and when
We also collect limited technical information automatically, such as your IP address, browser type, the pages you view and the page that referred you.
This form is not intended for medical information, so please do not include any. Medical information you share with us as part of your care is handled separately, as required by federal law (HIPAA), and is described in our Notice of Privacy Practices.
How we use your information
- To respond to your request and schedule your appointment
- To contact you by phone and email about your appointment, and by text only if you checked the optional box
- To send appointment confirmations, reminders and rescheduling notices
- To improve our website and our service
- To meet legal, accounting and recordkeeping obligations
Text messages (SMS)
Text messages are optional. Our appointment form carries an optional, unchecked SMS consent box that you can check to receive text messages from Fit 4 Life at the number you provide, including messages sent using automated technology. If you leave it unchecked, you can still submit the form and we will still contact you, but we will not text you.
Consent to receive text messages is never a condition of booking, of any purchase, or of receiving any service from us.
Message types: appointment confirmations, reminders,
arrival-time updates and rescheduling notices.
Frequency: message frequency varies with your appointments.
Cost: message and data rates may apply.
Opt out: reply STOP to any message. You will
receive one confirmation message and no further texts.

Carriers are not liable for delayed or undelivered messages.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
How we share information
We do not sell your personal information. We share it only as follows:
- Service providers. Companies that run our scheduling, customer relationship, email, telephone and hosting systems, only so they can provide those services to us.
- Legal requirements. Where required by law, subpoena or court order, or to protect our legal rights.
Mobile phone numbers and text message consent are excluded from all sharing for marketing or promotional purposes, including with affiliates.

Your choices

No method of transmission or storage is completely secure, so we cannot guarantee absolute security. Children
This site is not directed to anyone under 18, and we do not knowingly collect personal information from children through it. If you believe a child has given us information, contact us and we will delete it.
